Waiting Period 30 calendar days – Earlier reduced benefits may be payable based on lack of sick/personal leave down to zero days. $750 Minimum Benefit (see Minimum Benefits below) after 60 days, freeze of sick/personal leave after 60 days. No benefits are payable if working full-time, light or modified duty.

Non-Industrial: 30 calendar days

 

Industrial: 60 calendar days

ADEA Grading

A) Age 65 coverage (Non-Industrial causes).
B) Five year coverage with $60,000 lump-sum
payment (Industrial causes).
C) By signing a release of liability form, member is eligible for a $3,000 bonus plus $200 for each month
of integration within the first 24 months of claim.
Freeze of Sick Leave Options After 60 days, you may use 50% sick/personal leave and receive a 50% benefit from the Plan or use 100% of sick/personal leave and get $750 per month. After 60 days or 100% back door integration – 100% of base pay through use of sick/personal leave pay and LTD benefit. None

Cost of Living Benefit (COLA) 4% compounded per year (years 2-7) thereafter, CPI increase to age 65 and then benefits continued lifetime.

Stress & Substance Abuse Three (3) months per occurrence, six (6) month lifetime benefit. A Participant must return to work for one (1) year between claims. Benefits limited to 12 months. No limit if confined to a medical facility. Twenty-four (24) months.
Musculoskeletal & Connective Tissue Disorders Fully covered, no restrictions lifetime – Industrial & Non-Industrial causes 24 months, lifetime limitation on back and its surrounding soft tissue, sprains, strains of joint or muscles, and connective tissue disorders. Numerous conditions are limited to 24 months. See items A, B, & C above.

After 60 days – 70% of wages or maximum benefit of $9,000 monthly (Up to 70% repayable if determined to be Industrial disability). After no-pay status has been established 66 2/3% of wages or maximum benefit of $6,000 (Up to 66 2/3% repayable if determined to be Industrial).

After 180 days of continuous disability, eligible dependents will receive a lump sum benefit equal to six (6) times the member's last LTD monthly benefit, after reductions by deductible income.

Twenty-four (24) month continuation of benefits if member was eligible for disability benefits at time of death.
Death Benefit $15,000 Death Benefit on- or off-duty natural, accidental or terminal illness (Payable and delivered usually within 24 hours of notification). $15,000 (CSFEWBC Self Funded) $50,000 accidental death only.
Not paid on natural deaths.
