To update your beneficiary,
click here to download a PDF document. Print the new card, fill in the required information and mail it to:
CAPF, P.O. Box 31, Martell, CA 95654-0031 Your beneficiary information will be updated as
soon as we receive your card.
Visit the Adobe home page to download Adobe Acrobat Reader, or versions for other operating systems such as Macintosh or Unix .
Call the CAPF administrators, California Administration
Insurance Services, Inc., toll-free at (800) 832-7333 and ask for a change of beneficiary card. We will mail a new card to you for you to complete and return to us.
